---ALL HOT WORK PERFORMED ON THE GSU CAMPUS REQUIRES A PERMIT---
Complete the top section of the permit. The Authorized Person is a GSU employee who has been trained in Hot Work and has the authority to carry out the work. Include specific descriptions of the job being performed and the location. List all persons performing the Hot Work.
- Check off all of the listed conditions that apply. If any items are checked, a Fire Watch is required for the duration of the Hot Work, and 30 minutes after the work is completed. If a Fire Watch is required, include that person’s name.
- Inspect the work area thoroughly and check off each applicable precaution that is taken. If a precaution applies to the work, it must be taken and checked off. If a listed precaution does not apply, leave it blank. If additional precautions are warranted that are not listed on the permit, write them in at the bottom of this section.
- Once all applicable precautions have been taken and checked off in Section 2, the Authorized Person must verify this with his/her signature and the date. The permit expiration date and time must be included. A permit is good for only one day (24 hours) unless authorized by Environmental Health & Safety. Once this section is complete, the work may commence. The permit is to be posted at or near the job site for the duration of the job.
- When the Hot Work is completed, the Authorized Person must inspect the work area and verify that the work is complete and no fire hazards exist. The Authorized Person must then sign and date the permit and return it to his/her department to be filed.
